From the 16th to the 18th century, there was a disease killing more British sailors than all naval battles combined did. This silent threat was not an enemy fleet or a storm at sea—it was scurvy, a devastating illness caused by a lack of vitamin C. Without fresh fruits or vegetables on board, sailors’ bodies slowly broke down. Teeth loosened and fell out, gums bled, joints ached, and energy drained. In the worst cases, men collapsed or died. Some even suffered hallucinations so vivid they would leap overboard, believing the sea was a green field.
Scurvy haunted long sea voyages. It’s estimated that it claimed the lives of over two million sailors during the Age of Sail (*). Though sailors and captains suspected that citrus fruits helped, medical knowledge of the time lagged behind. Theories abounded—bad air, salty meat, or poor hygiene—but none pointed clearly to the real cause: vitamin C deficiency.
In 1747, a Scottish naval surgeon named James Lind made a breakthrough. Aboard HMS Salisbury, Lind conducted one of the first recorded clinical trials. He took twelve sailors suffering from scurvy and divided them into six groups, each receiving a different treatment: vinegar, seawater, a nutmeg paste, and so on. Only the group given oranges and lemons recovered quickly—one was well enough to return to duty after just six days.
Lind published his findings in A Treatise of the Scurvy in 1753, but his message was buried among outdated theories. For decades, little changed. It wasn’t until 1795 that the British Royal Navy, guided by physician Gilbert Blane, officially adopted daily lemon juice rations for sailors. The results were extraordinary. Scurvy virtually vanished from British ships, giving the Royal Navy a tremendous advantage during its long campaigns, including those against Napoleon.
Though scurvy became a symbol of outdated maritime hardships, the disease never truly disappeared. In recent years, it has resurfaced—even in developed countries. Doctors have reported modern cases among people with extremely poor diets, often lacking fresh fruits and vegetables. One man in Australia, for example, developed scurvy after months of eating little more than processed food.
Scurvy’s story is more than just a tale from the past. It’s a lesson in the importance of nutrition, the value of scientific testing, and the danger of ignoring evidence. James Lind’s experiment was one of the first steps toward modern clinical trials. And the Royal Navy’s eventual use of citrus juice was an early success story in public health policy.
Today, scurvy reminds us of how something as simple as a lemon could save a life—and how listening to science can change the course of history.
